USAir Flight 5050 crashes into Bowery Bay after takeoff at LaGuardia, killing two and injuring 21

The NTSB determined that multiple errors on the part of the captain led to the crash. The pilots union sequestered the plane's pilots for nearly 40 hours away from investigators, delaying drug and alcohol tests.
